A recent survey found that 36% of University of Pittsburgh Medical Center workers are in medical debt to their employer.
UPMC is a healthcare giant. It employs more than 92,000 people, has an operating revenue of roughly $26 billion, and as a “non-profit”, it’s technically supposed to provide affordable healthcare to the people who need it most.
But its flagship hospital recently made headlines for being the least charitable non-profit hospital in the country.
In 1969, the tax code was modified so that a hospital didn't have to pay taxes as long as it was “promoting health.” Charity care" became optional.
So Vikas Saini and his colleagues at the Lown institute decided to take a closer look at just how far some non-profit hospitals have strayed from their original mandate of giving back. They looked at over 1,700 nonprofit hospitals nationwide and calculated what they call the “fair share” spending for each. If a hospital took more money in tax breaks than it gave back to the community, it had a fair share deficit.
Meanwhile, UPMC’s former CEO made $12.9 million in 2021. In total, top executives raked in $225 million — more than double what UPMC spent on charity care that year.
That’s enough money to keep hundreds of rural hospitals open, or to cancel the medical debt of hundreds of thousands of patients.
In just the last ten years, UPMC has grown from 12 hospitals to over 40.
Understaffing is pervasive at UPMC — and it’s only become worse as the hospital has gained more and more market power.
In May 2023, a coalition of unions filed an antitrust complaint with the Justice Department, accusing UPMC of using its market power to harm workers.
Three quarters of nonprofit hospitals are getting more in tax breaks than they’re spending on communities. That’s enough money to cancel the medical debt of 18 million Americans.

Olga González believes nonprofits often underestimate the communities they serve. They swoop in, she says, with outside resources and money but ignore the assets these communities already possess. That's why she endorses the promotora model, where a nonprofit trains people from the community they serve to provide outreach, advocacy, and services. Here, González shares several success stories of promotoras in action – from distributing masks & vaccines to farmworkers during the COVID-19 pandemic to monitoring air pollution near a local oil refinery. Olga González has worked in the nonprofit sector for 28+ years. She is currently the Executive Director of Cultivando, a nonprofit that cultivates leadership in the Latinx community to advance health equity through advocacy, collaboration, and policy change. As CEO of O.G. Consulting Services, she facilitates trainings and workshops on equity, racial justice, and healing. Olga was born in Mexico and is the proud descendant of Indigenous Yaqui/Otomi people. She, along with her husband, is raising their children to be the next generation of fierce and compassionate social justice warriors. Casey Putsch speaks out about the abuse of non-profit entities and charity in the United States as well as the societal and cultural impacts of this corruption to society. Casey has spent 11 years volunteering his time in a 501(c)3 educational charity for which he has been honored to fly with the United States Air Force Thunderbirds as well as with a Lifetime Achievement Award from President Trump and President Biden. When looking at Non-profits and where they fit into a community, the importance of planning and evaluation can not be overstated. Nonprofits serve the community in many different ways and have many different stakeholders. Stakeholders being anyone involved in the nonprofit or being affected by the nonprofit. Planning and evaluation allows the stakeholders to know the Non-profit is working to be a good steward of the resources they are working with and the services they provide. Planning and evaluation also allows the nonprofit to pivot accordingly when situations change, and it ensures the nonprofit is being accountable to it stakeholders. Gordon Decker is a 20-year law enforcement veteran and 12-year Army National Guard veteran. He holds a Masters Science in Administration with an emphasis in Organizational Leadership from the University of South Dakota and has more than 8 years of experience working with non-profits of various sizes and at different levels of responsibility. He currently owns his own business and was previously a product manager for a software company. Gordon has been married 14 years and has two children (9 and 12 year old boys). He prides himself on being a lifelong learner, passionate about growth and development personally, professionally, and as a community. At the Techonomy conference in San Francisco, Bill Gates closed the event and answered questions from the audience. One woman asked why non-profits can't behave more like for-profit businesses. Gates said that wasn't necessarily the best idea.
